Please fill out the Youth & Community Outreach Fund Request form and
submit it along with a letter from your organization (on company letterhead)
describing the event and/or activity in detail and how the requested
funds will be utilized. If your organization is a 501(c)3, you must also
submit a copy of your IRS Certification verifying your status as a
not-for-profit organization.

The form will need to be signed and dated. Therefore email copies will
not be accepted.  Please mail or deliver the required Youth and
Community Outreach Fund Request and supporting documents to the County
Manager's Office at 96135 Nassau Place, Suite 1, Yulee, Florida 32097.
Once received, your request will be scheduled for the first available
Board of County Commissioner's Meeting for consideration.
